The CDA Dental Wellness Membership is NOT insurance. The program is only offered to individuals and families not currently insured by a dental health plan. The program benefits are not transferable to another dental practice or dental specialty. Membership is for 1 year beginning on the enrollment date. Membership will AUTOMATICALLY renew on the anniversary date of the contract unless written request to cancel is received 7 days prior to renewal. A reminder email will be sent 14 days prior to renewal. Membership dues are payable in full upon enrollment and are non-refundable. Membership must be current to receive the discount. It is the members responsibility to schedule and keep all appointments offered as part of the dental program. Please notify our office at least 48 hours in advance if you must change a scheduled appointment or a $50 fee may be applied. Payments for patient portion of any dental treatment are the members responsibility and due on the date of service to qualify for the discounts.
